Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses to examine the effect of treatment across trials of various levels of pragmatism.
Background
Pragmatic trials are becoming more widely recognized as providing real-world evidence for clinical decision-making. However, the usage of the term "pragmatic" is not uniform and its definition and evaluation requires clarification. The purpose of pragmatic trials is to inform clinical practices and policy decisions, not to confirm a physiological hypothesis or clinical hypothesis. A pragmatic study should aim to be as similar to the real-world clinical environment as possible, including in its recruitment of participants, setting up and design, the delivery and execution of the intervention, as well as the determination and analysis of outcomes as well as primary analyses. This is a key difference from explanatory trials (as described by Schwartz and Lellouch1) which are designed to provide more complete confirmation of a hypothesis.
Trials that are truly pragmatic must be careful not to blind patients or clinicians in order to cause distortions in estimates of the effects of treatment. Despite these criteria however, a large number of RCTs with features that defy the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all kinds. This can result in misleading claims of pragmatism and the use of the term must be standardized. The development of a PRECIS-2 tool that provides a standardized objective evaluation of the pragmatic characteristics is the first step.
Methods
In a pragmatic trial, the aim is to inform clinical or policy decisions by demonstrating how an intervention would be integrated into everyday routine care. Explanatory trials test hypotheses concerning the causal-effect relationship in idealized environments. In this way, pragmatic trials can have a lower internal validity than explanatory studies and are more susceptible to biases in their design analysis, conduct, and design. Despite these limitations, pragmatic trials may provide valuable information to decisions in the context of healthcare.
The PRECIS-2 tool evaluates an RCT on 9 domains, with scores ranging from 1 to 5 (very pragmatic). Thus, they are not very close to usual practice and can only be described as pragmatic in the event that their sponsors are supportive of the absence of blinding in these trials.
Additionally, a typical feature of pragmatic trials is that researchers try to make their results more relevant by analyzing subgroups of the sample. This can lead to unbalanced analyses that have lower statistical power. This increases the chance of omitting or misinterpreting differences in the primary outcomes. In the instance of the pragmatic trials included in this meta-analysis this was a significant problem since the secondary outcomes weren't adjusted for differences in baseline covariates.
Additionally, pragmatic trials can also present challenges in the gathering and interpretation of safety data. This is due to the fact that adverse events are typically self-reported, and therefore are prone to delays, inaccuracies or coding errors.
