Pragmatic Free Trial Meta is a free and non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 allowing for multiple and diverse meta-epidemiological studies to examine the effects of treatment across trials that have different levels of pragmatism, as well as other design features.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision-making. The term "pragmatic" however, is not used in a consistent manner and its definition and measurement need further clarification. The purpose of pragmatic trials is to guide clinical practice and policy decisions, rather than to prove the validity of a clinical or physiological hypothesis. A pragmatic trial should try to be as close as it is to actual clinical practices which include the recruitment of participants, setting, designing, delivery and implementation of interventions, determining and analysis outcomes, and primary analyses. This is a key distinction from explanatory trials (as described by Schwartz and Lellouch1) which are intended to provide a more thorough confirmation of a hypothesis.
Studies that are truly practical should be careful not to blind patients or healthcare professionals, as this may lead to bias in estimates of the effects of treatment. Pragmatic trials should also seek to enroll patients from a variety of health care settings, to ensure that the results can be compared to the real world.
Furthermore studies that are pragmatic should focus on outcomes that are important for patients, such as quality of life or functional recovery. This is especially important for trials that involve invasive procedures or have potentially dangerous adverse consequences. The CRASH trial29 compared a 2 page report with an electronic monitoring system for hospitalized patients with chronic heart failure. In addition to these aspects pragmatic trials should also reduce the procedures for conducting trials and requirements for data collection to cut costs and time commitments. Finaly, pragmatic trials should aim to make their results as relevant to actual clinical practices as possible. This can be achieved by ensuring their primary analysis is based on the intention-to treat method (as described within CONSORT extensions).
Despite these requirements, many RCTs with features that challenge pragmatism have been incorrectly self-labeled pragmatic and published in journals of all kinds. This can lead to false claims of pragmatism, and the usage of the term should be standardized. The development of the PRECIS-2 tool, which provides an objective and standard assessment of practical features is a great first step.
Methods
In a pragmatic study it is the intention to inform clinical or policy decisions by demonstrating how the intervention can be integrated into everyday routine care. This is different from explanatory trials that test hypotheses regarding the causal-effect relationship in idealized conditions. In this way, pragmatic trials may have lower internal validity than explanation studies and be more susceptible to biases in their design analysis, conduct, and design. Results
